Facial Feminization Surgery – Before and After

This patient underwent the following procedures in one stage as part of Facial Feminization Surgery (FFS).
– Hairline lowering – The hairline lowering aimed to reduce the overall size of the non-hair-bearing portion of the forehead. This improvement is visible in the front, three-quarter, and lateral views. Note the quality and visibility of the scar.
– Type 3 brow bone reduction – The outcomes of the type 3 brow bone reduction are most apparent in the front, three-quarter, and lateral views. Across these views, the increased openness of the eyes is evident, with reduced shadowing and a less deep-set appearance. Overall, this significantly softened the upper third of the face.
– Orbital contouring
– Coronal brow lift – When a dramatic brow prominence is reduced, a coronal brow lift becomes necessary to reposition the eyebrows above the orbital rim. This is best observed in the front and three-quarter views.
– Rhinoplasty (Dorsal reduction, nasal bone narrowing, increase in tip definition) – The rhinoplasty outcomes should be evaluated from all angles. The dorsal hump reduction is most noticeable in the lateral and three-quarter views, where the nasal dorsum exhibits a gentle slope from the radix down to the nasal tip. The nose’s frontal view demonstrates the appropriate contour of the dorsal aesthetic lines (left and right edges of the nasal dorsum). Finally, the worm’s-eye view of the nasal tip confirms the absence of significant tip deviation or asymmetries.
– Jaw contouring
– Chin reduction – This patient had significant forward projection of the chin preoperatively. Following reduction of the chin, the results are most evident in the lateral views. The three-quarter views also illustrate a softening of the chin prominence. The remaining jawline was contoured to achieve a smooth transition from the chin to the posterior jaw.

Dr. Mittermiller’s approach to facial feminization is to create a result that appears inherently feminine and natural. These before-and-after photos allow patients to observe the significant visual transformation possible through targeted FFS. Because facial structure and personal goals vary, discussing your specific anatomy and expectations during a private consultation is essential.
