These before and after photographs of a feminizing rhinoplasty demonstrate some of the key priorities that one often hopes to accomplish with a feminizing rhinoplasty in FFS (Facial Feminization Surgery).

This wonderful patient had a prior reduction rhinoplasty by another surgeon. It appears to have been a challenging primary rhinoplasty. She was left with a small residual dorsal hump, a large hanging columella, and tip asymmetries. The dome of the right nasal tip had much less projection compared to the dome on the left.

A revision rhinoplasty was performed to correct these issues. The prior stair-step incision was used for access. The patient’s septum had previously been used during her primary rhinoplasty, so rib graft was harvested during this operation to provide additional structure to the nose.

Dr. Mittermiller’s approach to a feminizing rhinoplasty is to create a nose that appears feminine and natural.
