Facial Feminization Surgery (FFS) Before and After Photographs


These before and after photographs of facial feminization surgery demonstrate the effects that can be obtained with a brow bone setback (type 3), orbital rim contouring, brow lift, and surgical hairline lowering.
This wonderful patient underwent forehead feminization as part of her first facial feminization operation and is only 2 months postop!
The dramatic reduction in the prominence of her orbital rims is seen in the three-quarter view (first image set). This procedure reduces the ledge over the eyes and allows more light to fall on the eyes. It gives the appearance of “opening the eyes.”
The brow bone reduction in the pure lateral view (second image set) shows the significant yet natural reduction in the brow prominence.
The hairline lowering is still healing, this early in the process. At this point in the recovery, the scabbing is mostly gone, but patients frequently have a visible scar from the pink color. This will slowly fade over the next few months.

The term “forehead reduction” can sometimes be misleading depending on how the term is used. Sometimes the term is used to refer to the reduction of the bony part of the forehead. Other times, the term “forehead reduction” can refer to reducing the size of the non-hair bearing skin between the eyebrows and the hairline. Dr. Mittermiller prefers to avoid the term “forehead reduction” because it can be confusing when used out of context. Instead, he prefers to use the terms “brow bone reduction” and “surgical hairline lowering” to differentiate the two procedures. This patient underwent both procedures as part of her surgical journey.
