Facial Feminization Surgery (FFS) Before and After Photographs

These before and after photographs demonstrate the intraoperative results of a hairline lowering, type 3 brow bone reduction, orbital rim contouring, brow lift, and feminizing rhinoplasty as part of facial feminization surgery (FFS).

This is always a powerful combination of procedures. 

The hairline lowering uses an incision in the front the hairline to bring the hairline lower on the forehead, to create a more rounded hairline, and to reduce the areas of temporal recession. It does create a scar high on the forehead, so this needs to be carefully considered by patients who are interested in the procedure. The scar fades with time, but it will always be present.

A type 3 brow bone reduction involves creating cuts in the forehead bone in order to maximally reduce the brow ridge prominence. This is commonly performed in combination with orbital rim contouring, which reduces the shelf of bone above the lateral eyes.

The brow lift is particularly enhanced when performing a hairline lowering with the brow lift. By creating tension on the brows, they can be maximally elevated. The brows do come down with time, but the effect is still notable. 

Dorsal reduction rhinoplasty is one of the most common requests for facial feminization and they provide a big impact on the appearance of the face. There is often a small amount of variation in hump reduction and tip rotation that still falls within a natural-appearing result. In addition to improving the feminine appearance of the nose, Dr. Mittermiller also pays careful attention to improve the natural appearance of the nose. For example, this patient had a tip asymmetry that she was unaware of prior to surgery and was corrected at the time of the operation.
