This patient underwent rhinoplasty with multiple techniques. He underwent a dorsal reduction, placement of spreader grafts to improve the airway, septoplasty, and turbinate reductions.

With a male rhinoplasty, it is important to carefully maintain tip position. Men usually do not want a droopy tip, but they also do not want a tip that is too high since this can appear feminine. Additionally, a straighter nasal dorsum as seen in the profile view is often desired for men. If the dorsum is reduced too much, this can also produce a feminine appearance.

Creating a straight nose as seen from the front can be very challenging due to the complexities of the nose. This patient underwent a unilateral nasal osteotomy to widen the nose on one side and asymmetric placement of spreader grafts to straighten the nose and widen the nose on only one side.
